The Town Clerk can provide certified copies of all birth, death and marriage records that are on file with the Town.  The Massachusetts State Registry of Vital Records & Statistics has records for ALL Towns / Cities in Massachusetts. 

The Town of Truro will only have the following Vital Statistic Records on file:

• Birth records:  Anyone born in Truro or whose parents were living in Truro at the time of the birth. 
• Death records:  Any death within the town of Truro or if the deceased was a resident of the town of Truro at the time of death.
• Marriage records:  Any couple who filed their intentions to be married in the town of Truro  Please note that even if the marriage took place in Truro, we will not have the record unless the marriage intentions were filed here.

Certified copies of birth, death or marriage certificates cost $10.00 for the first copy and $10 for each additional copy ordered. 

Obtaining a Vital Record
You can request certified copies of a vital record in person, by mail, by phone or email.  Please be prepared to provide the name of the person, the type of record you are requesting, the event date and where the event occurred by completing the application for a Vitals Records Request Form. 

Funeral Home Information:

Funeral Home

If you are using a funeral home for arrangements, the funeral home will gather all of the information and enter it into the MA Registry of Vital Records and Statistics.

No Funeral Home

If you are not using a funeral home for arrangements, information will need to be provided to the Town Clerk to enter into the MA Registry of Vital Records and Statistics. Whomever is responsible for the arrangements will need to submit an Informant Worksheet for Certificate of Death (PDF). The attending physician will also need to submit a Death Certificate Medical Certifier Worksheet (PDF) unless they are able to submit it directly to the MA Registry of Vital Records and Statistics.
